Responsibility

you will be part of a team of Qualified Senior Support workers who will be responsible for
people experience compassion, dignity and respect in their everyday life
development and review of outcome focused support planning, working alongside external professionals and families
supporting those affected by homelessness and addictions
inclusion of carers, friends and family members in agreed interventions and expected outcomes
carrying out welfare checks
developing relationships with the persons wider social circle to encompass them into the person's life
managing a case load of individuals
to assess, plan and review risk assessments and support plans in a risk adventurous manner
to be responsible for undertaking all social work on allocated cases, or those worked with on a short term assessment basis
managing risk and safeguarding
to produce case reports and correspondence for a variety of readership, including meetings, conferences, court, other agencies and service users
to provide supervision, as appropriate, to social workers, students and/or social work assistants
establish appropriate professional relationships with service users, their families and other professionals to enable effective partnerships in the provision of services
to undertake cases with complex risk levels
make on-going assessments of allocated cases to reflect individual circumstances including assessment of risk, enabling independent functioning, and need to ensure that service users are protected from significant harm
prepare and present reports as necessary to facilitate proper decisions being made in respect of service users and to provide quality assurance
actively contributing to team working, supporting colleagues, covering for colleagues in their absence and taking part in team meetings
carrying out duty work; making referrals and Assessments
to Support the Registered Manager with the day to day running of the home
