  • A skilled healthcare professional with a passion for making a difference in the lives of individuals and families
  • registered Nurse or Health Visitor with relevant qualifications
  • proven experience as a health visitor, with a strong understanding of community healthcare practices
  • excellent communication and interpersonal skills, with the ability to establish rapport and build trust with patients and their families
  • sound knowledge of health promotion, disease prevention, and early intervention strategies
  • strong organisational and time management abilities to prioritize and manage caseload effectively
  • ability to work independently and collaboratively within a multidisciplinary team
  • passionate about community healthcare and possess the necessary qualifications and skills
  • qualified Health visitor with EPR experience
  • we expect the practitioner to demonstrate at least 2-year prior experience working with children and/ or adults with a learning disability

Responsibility


  • monitor and evaluate the effectiveness of care plans and make necessary adjustments to ensure positive outcomes
  • conducting home visits to assess the health and development of children, providing support and guidance to parents and caregivers
  • delivering evidence-based interventions and health promotion activities to improve the overall well-being of families
  • providing information and advice on child development, parenting skills, immunizations, and nutrition
  • collaborating with multidisciplinary teams and partner agencies to ensure comprehensive care and effective referrals when needed
  • maintaining accurate and up-to-date records, including electronic health records, to track patient progress and outcomes
  • participate in professional and family meetings where appropriate
