Responsibility
- carrying out Initial Care Assessments, ensuring all care needs are met through outcome focused support plans
- ensure care delivery is appropriately organised within rota’s and staff availability
- ensure regular reviews are carried out and documented
- carry out regular Quality/ Contact calls with Service users
- maintain clear and accurate administration documentation, including accident & incident reports, using digital system and paper files
- participate in on Call on a Rota basis
- deliver care service when necessary
- ensure Health and safety is adhered to at all times by carrying out Risk Assessments, Manual handling assessments and updating on an ongoing basis
