Responsibility
- conduct assessments and reviews in line with the Care Act 2014, including those for individuals outside the area
- develop care and support plans that meet the needs of individuals and their caregivers
- handle section 42 Safeguarding enquiries, adhering to Making Safeguarding Personal principles and coordinating Safeguarding meetings
- embrace a 'strengths-based' approach, promoting independence and utilizing community resources when possible
- perform Mental Capacity assessments and, where needed, complete Best Interests Decisions under the Mental Capacity Act 2005
- evaluate risks and work to minimize them
- job title: Social Worker
- assess cases of self-neglect, sometimes taking a lead role, in line with multi-agency policies
Requirements
- qualified social worker
- experience in a similar setting to the applied-for post
- registered with appropriate statutory body
- proficiency in a wide range of social work and social care duties, including duty tasks
- successful completion of ‘Assessed and Supported first Year of Employment’ for those with one year or more of post-qualifying experience
- experience conducting s.42 safeguarding enquiries
- relevant experience in social care with older adults, physically disabled individuals, or people with learning disabilities
- proficiency in undertaking mental capacity assessments and following Best Interests processes
