Responsibility
- build strong links with the multi-disciplinary teams within each GP surgery
- offer referred patients strength based one to one support, either remotely or in person, building boundaried, non-judgmental and impactful connections
- proactively encourage volunteering opportunities and access to local community resources, enabling peer to peer support, good neighbourliness and ‘active citizenship’
- introduce and if required, accompany patients to groups, clubs, and meetings they would like to try
- as a key member of the PCN multi-disciplinary team, you will provide personalised support to patients, their families and carers in order to improve health and wellbeing
- accrue and make best use of local knowledge regarding local health, social care and voluntary provision to support patient care and independence
- ensure an empowerment model is employed to reduce patient service dependence
- following initial assessment, create personalised plans that focus on improving health and wellbeing and show the outcomes the patient wishes to achieve
Requirements
- proficiency with office 365 and CRM systems
- confidence with face to face, telephone and with email communications
- knowledge of the voluntary and community sector, and health & social care systems in the Wokingham Borough
- effective communicator and team player
- ability to prioritise workload and respond effectively to competing demands
- solution focused and able show initiative and take responsibility
- ability to work independently, with GP surgery staff, and as part of the Involve Community Services team
- punctual and effective time management
