Responsibility
- elite Homecare Service is looking for a dynamic, experienced Care Coordinator to fulfill a permanent position within our friendly inclusive team
- to organise and coordinate care staff to travel to the residence of our clients
- to ensure care staff complete all tasks required by the client, as specified in their individual Care Plan
- to liaise with family, relatives, local authority social workers, district nurses and other members of the care team
- to inform care staff of changes to the care plan prior to care staff arrival
- to document and report to the Registered Manager on the welfare of the client
- in cases of staff shortages and in emergencies act in the role of Care Worker ensuring pre-arranged care visits take place
- to cover emergency oncall responsibilities on a rota basis
Requirements
- vehicle owner/driver
- relevant qualifications for the role of a coordinator
- basic geographical knowledge of the following areas; Hartlepool and the surrounding villages
- level 3 Administration of Medication
- good knowledge of the Care Sector
- previous experience working as a coordinator
- previous experience working in a care office
- providing care: 1 year
