Responsibility
- as a Care Coordinator, you will be responsible for managing and coordinating the care services provided to our Service Users
- coordinate and schedule client appointments and visits
- coordinate and manage the day-to-day delivery of care services to Service Users
- maintain accurate records of client information and care plans
- develop, review, and update personalised care plans in consultation with Service Users and their families
- communicate with Service Users, their families, and healthcare professionals to ensure proper coordination of care
- schedule and allocate care workers to meet the needs of Service Users, ensuring continuity of care
- assist in the recruitment and training of care staff
Requirements
- previous experience in an office or administrative role is required
- knowledge of healthcare or social care industry
- excellent communication skills, both written and verbal
- strong organisational skills with the ability to multitask and prioritise workload
- ability to work independently as well as part of a team
- be able to drive and have use of your own car
- providing Home Care: 1 year
- management: 1 year
