training, Education & Information Deliver supervision to clinical pharmacists Deliver education and support to individual patients during own clinics Provide ad-hoc clinical advice to doctors and other clinicians Assess emergent clinical evidence and develop strategies for implementation in practice
job responsibilities Responsibilities may include but are not limited to: Work alongside the PCN Manager, Clinical Director, and PCN Board to deliver the supervision plan for the Clinical Pharmacist Academy Provide line management to all clinical pharmacists in the PCN Liaise with key stakeholders within the PCN and Nottingham University Run some own clinics to support the practices An average week will look like: Monday - AM admin / PM Skills workshop for trainee ACPs Tuesday - Supervise 2 x trainee ACPs all day Wednesday - Supervise 2 x trainee ACPs all day Thursday - AM same day clinic / PM admin and Board meetings Friday - Supervise 2 x trainee ACPs all day Key Objectives Provide proactive supervision and leadership to the clinical pharmacist academy Develop strong working relationships with Nottingham University, other members of Rutland PCN, and the member practices to ensure students are correctly supported Plan and manage the clinical pharmacy workload When running your own clinics: Assess and triage patients, including same day triage, and as appropriate provide definitive treatment and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person Safety Report medicines related incidents, contribute to investigations and root cause analyses Participate in serious incident investigations and multidisciplinary case reviews Report Adverse Drug Reactions and promote reporting by MDT Action and respond to Drug Safety Alerts and other safety cascades
