- oversee performance of contracts, quality delivery and achievement of outcomes ensuring high levels of Carer, volunteer, commissioner, stakeholder, and staff satisfaction
- gain an understanding of their caring role and develop individual support plans that reduce the impact of their caring responsibilities

- make appropriate referrals for a statutory Young Carers Assessment

- work closely with the Services Director and the Finance Director to set, manage, and monitor the service budgets

- the role is to support Carers to review and reassess their Carers Assessments and Peace of Mind for Carers contingency plans in line with local authority procedures
- contact newly referred Carers to induct them to the service and to establish their needs
- can you support unpaid Carers by reviewing their needs?
- the role is to cover the Rochdale area, and proactively identify Carers, provide them with information, advice and support including supporting them to access services, peer support and short break opportunities in order to reduce the impact of their caring role
- participate in multi-agency meetings such as team around the family, child protection conferences and child-in-need reviews

- we are looking for people who are passionate about supporting unpaid Carers to reduce the impacts of their caring role by providing professional and quality support
- where there have been no changes since their assessment, ensure the Review Form is complete enabling the Carer to continue to receive the same levels of support
- geographical coverage: North, Central and West Lancashire
- ensure POM4C plans are registered with the call monitoring provider for the POM4C Scheme
- work as part of the overall Services Leadership Team and support the Service Director in n-compass wide initiatives and developments
- we are looking for people with; the ability to listen and communicate effectively with a wide range of individuals; experience of sensitively discussing and gathering information from people who want to access support; and the ability to provide individualised 1-1 support to adults
