- record evidence of positive and negative outcomes from the delivery of care and inputting data monthly
- we have an opportunity for a Care Coordinator to join the team based in Leeds. You'll join us on a full time, permanent basis, and in return, you will receive a competitive salary of £21,000 - £23,000 per annum

- make recommendations and changes to the care plan whilst involving clients
