Responsibility
- you will have the ability work positively alongside a team of support workers and Housing staff
- you will primarily work within the client’s home but may also work in other appropriate community settings
- you will be helping a small caseload of individuals to progress towards independence and independent living
- the overall aim of this role is to support CC Housing Management Ltds wider vision in reducing the total number of people homeless
- devise and implement suitable support plans to ensure that identified support needs are met
- clients will be supported to identify and focus on realistic and achievable short-, medium- and long-term goals
- encourage and support clients to live within the local community, providing information, emotional, organisational and practical support, as appropriate
- under the leadership of the Senior Support Worker and Supported Housing Manager to assist in the provision of effective, high quality support services to vulnerable, single homeless individuals who have needs such as offending behaviour, substance misuse problems, mental health issues and who are vulnerable to rough sleeping with the aim of promoting independent living
