Responsibility
to coordinate and develop peer support for young carers and their families including facilitating a range of groups and activities
assess young carers needs and support young carers to identify their caring needs
coordinate appropriate support to young carers dependent on identified need
regularly contribute to the setting of and measure the outcomes for young carers using the Outcome Star
the Young Carers Support Worker will contribute to the delivery and planning of activities for young carers, aged 5-18
they will also refer and signpost to other appropriate support services and identify and assist previously unidentified carers through a variety of mediums including schools, community provision, social services, GP surgeries and other youth services
the Young Carers Support Worker will also help to assess carers needs and identify what level of support they require from our services or other appropriate services in the Slough area
to continually raise awareness of young carers and the issues they face within the school setting e.g. presenting at assemblies
