Responsibility
- supporting Care is an expanding leading home care provider in London since 2011
- we are currently recruiting for a Care Coordinator to maintain and develop our home care services
- schedule and coordinate client appointments and visits
- communicate with clients, their families, and healthcare providers to ensure effective care coordination
- assist in the development and implementation of care plans
- monitor and evaluate client progress and make necessary adjustments to care plans
- creating, managing, and monitoring customer care plans, ensuring staff rotas meet service users’ needs and aspirations
- provide support and guidance to clients and their families regarding available resources and services
Requirements
- experience in Care Coordinating minimum of 1 year
- knowledge and understanding of Dols/Mental Capacity Act/Safeguarding
- preferably NVQ level 2 or similar qualifications
- proven experience in a healthcare or social services setting
- excellent organizational and time management skills
- strong communication and interpersonal skills
- ability to work independently as well as part of a team
- compassionate and empathetic nature
