A basic understanding of the Recovery Model
Responsibility
working holistically with service users referred to Pathfinder services and engaging with them to achieve recovery objectives and goals, maximise independence and quality of life • To support the assessment of service users referred to Pathfinder services where appropriate
be fully aware of the needs of service users over a 24 hour span of care
adhere to Pathfinder policies and local service protocols
to support individuals and the Multidisciplinary team to meet identified rehabilitation and recovery goals with clients who have a range of complex mental and physical health needs and risks
to assist in continually improving and developing Ashness Care services
to work with service users who have identified rehabilitation and recovery needs as a result of mental health conditions and who may also have a range of complex physical health needs
