Responsibility
- working holistically with service users referred to Pathfinder services and engaging with them to achieve recovery objectives and goals, maximise independence and quality of life • To support the assessment of service users referred to Pathfinder services where appropriate
- be fully aware of the needs of service users over a 24 hour span of care
- adhere to Pathfinder policies and local service protocols
- to support individuals and the Multidisciplinary team to meet identified rehabilitation and recovery goals with clients who have a range of complex mental and physical health needs and risks
- to assist in continually improving and developing Ashness Care services
- to work with service users who have identified rehabilitation and recovery needs as a result of mental health conditions and who may also have a range of complex physical health needs
