- this diverse role allows the post-holder to work autonomously in the community to provide specialist assessment of patients with complex needs, developing management options and evaluating plans in the face of the changing needs of our patients
- the successful candidate will work within a multi-disciplinary team, developing good communication between patients and families and the nursing staff to maintain patient well-being and comfort
- whilst recent palliative care experience is desirable, we require applicants to have an understanding of hospice and specialist palliative care, and experience of working as part of a multi-professional team
