- the purpose of the Senior Nurse Coordinator role is: To be responsible for the day-to-day coordination of a high quality and integrated nursing service that will enable patients with life limiting illnesses to be cared for at home, and to die at home if that is their preference To assist in the ongoing development of the Hospice at Home Service To assist in setting the required standards of care and monitoring that these standards are achieved To assist in the recruitment, training and support of Hospice at Home staff Plan and organise own day to day workload Make decisions to enable comprehensive and coordinated support of patients living at home To cover 2 clinical shifts per week
