Responsibility

  • visit patients at their homes in order to provide personalised care
  • work with patients who have certain kind of illness, for example, elderly people with dementia
  • assist with transportation, personal care and activities of daily living
  • discuss medicine with patients to ensure the medicine is taken correctly
  • provide companionship and personal care to patients or the elderly
  • reach out to patients and families to provide education regarding disease or illness
