Requirements
- appointments are subject to a satisfactory disclosure from the Disclosure and Barring Service
Responsibility
- support and manage Hospital Carer Support team members to deliver organisational goals and objectives
- undertake first line management responsibilities including Reviews and PDR’s
- ensure Integrated Discharge Team Manager are notified of any absences/changes that affect the service
- first point of contact to the Hospital Carer Support team; providing information, support and guidance to queries or problems in relation to individual cases within the hospital setting
- provide examples of impact of support to Carers using case studies and evaluation tools
- escalate issues or concerns to the Service Manager that have been raised from the Integrated Discharge Team or Carers within the hospital setting that are unable to be resolved at a local level
- support the Service Manager in the implementation of operational change, revised practice, processes and reporting systems to facilitate continual improvement of the Carers Hospital Service
- contribute to Carers newsletters and promotional materials as required
