- you’ll be a part of a vibrant team of individuals dedicated to making a difference for older people
- we’re looking for a Service Manager to oversee our Falls Prevention programme in Swindon. This project aims to make a difference to those who are at risk of falls in the community
- recruit, manage and support a team of volunteers, to ensure the efficient and effective running of the falls prevention programme
- the main purpose will be supporting individuals to remain independent in their homes and empowering them to take charge of their health, wellbeing and independence, offering a wide range of support including home visits, new Fitness & Friendship clubs, transport and volunteer support
- identify gaps in existing service provision and shape and monitor referrals into the service
- develop, coordinate and deliver a falls prevention training/awareness programme, ensuring development of appropriate skills and competencies for staff and volunteers
- project manage the new Falls Prevention programme, developing and co-ordinating the programme, ensuring timescales for delivery are met. Monitor KPI’s and project successes
- manage the Falls Prevention Senior Project Worker, who will be conducting home checks and risk assessments with people at risk of falls
