- respond promptly to any client enquiries and be responsible for reviewing case load
- signpost and refer to the right support at the right time across a wide range of services
- carry out face to face, telephone or virtual appointments with an allocated case load
- carry out home visits to clients where required
- utilising a guided conversation approach to identify the most appropriate source of information and support and enable them to engage with activities, groups and other community programmes
- be the first point of contact at The Hub on a rotational basis, triaging referrals, allocating the IFA’s caseload
- carry out a person-centered strength-based assessment with the client, developing a care and support plan either face to face, virtually or telephone based
- provide information and support that helps clients stay connected in their communities and continue to lead meaningful lives, whilst living at home safely and securely
