- having a caseload of up to 18 foster carers and providing them with supervision on a monthly basis
- liaise with a team of Social Workers, case coordinators and health professionals
- manage an allocated social work caseload relating to children, young people and families
- foster carer supervisions and ongoing support to carers
- to provide direct support to young people leaving care through the provision of advice, guidance, and practical support
- work collaboratively with service users, their families, and other professionals to develop effective support plans

- ensuring assessment, support/care-plans and intervention is shared and agreed with relevant individuals and agencies

- filtering appropriate referrals for the carers
- promote and maintain independence for vulnerable adults
- completion of high-quality carer annual reviews

- to work collaboratively with other agencies and professionals to ensure the best possible support for young people leaving care

Requirements
- professional qualification in Social Work (Diploma in Social Work, BA/BSc/MA in Social Work, CQSW as required by Social Work England
- qualification in Social Work and must have completed the ASYE programme
- current registration with the HCPC
