Responsibility

  • these young people may have additional social, emotional, and mental health needs
  • principle Responsibilities and Duties
  • be an allocated key worker and manage a case load of service users
  • as a specialist support worker for Young People and Children First, you’ll be supporting young people between the ages of 16 and 25 who have been through the care system or have been affected by homelessness
  • carry out holistic assessments of service users; creating with service users, support plans and risk assessments, to achieve positive outcomes
  • work in partnership with professionals to meet the educational, employment, health and mental health needs of service users
  • you will work in a collaborative way with partner agencies to support our young people into independent living using our key working process to ensure positive outcomes
  • participate in, and complete reports for service user’s Statutory Reviews and meetings
