Responsibility
- be the first point of contact for all families/carers/professionals phoning and receiving visits from the service
- hold a caseload and be responsible for the initial assessment and review of the patients in this caseload
- line manage a team of health care assistants and band 4 coordinators with support from the community services manager
- work flexibly over both personal care service, hospice at home and other community services as required
- assist in the delivery of clinical care and associated activities in line with the appropriate standards and guidelines
