Responsibility
- key requirements of the role
- must be an Independent Prescriber
- delivery of structured medication reviews to those identified as being at highest need
- repeat prescription requests
- patient medication queries
- medicine reconciliation on transfer of care from other primary or secondary care settings
- supporting the quality improvement agenda as directed by the Quality Outcomes Framework
- management of minor ailments & self-limiting conditions
Requirements
- patients
- minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio
- GPs, nurses and other reception staff
- holds an independent prescribing qualification
- other external healthcare professionals including community pharmacists, pharmacy technicians, community nurses, optometrists, dentists, health and social care teams and dieticians
- ability to work as a member of a team within the PCN
- able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
- GP surgery: 1 year
