Responsibility
- we work closely with other healthcare providers including the NHS to ensure that our patients receive the highest possible care
- this care is given in the comfort of patients own homes by Weldmar Community Nurses and our Weldmar at Home team, through our Patient and Family Support Services team both in patients homes and at locations across Dorset or as inpatients at our hospice in Dorchester
- our team of dedicated staff and volunteers ensure that we can continue to support patients through one of lifes most difficult journeys
- we do receive some funding from the NHS but the majority of our income is generated by our fundraising and retail teams, which plays a vital part in securing income to fund our care, both now and in the future
- job Purpose To provide specialist clinical pharmacy service to palliative care patients within the Inpatient Unit, checking medication management, collaboration with the healthcare team, addressing medication-related concerns, and ensuring compliance with UK regulations and hospice guidelines
- work very closely with the discharge co-ordinator to provide the patients with a smooth and efficient discharge process
- to provide expert advice and support to the multi-disciplinary team at Weldmar Hospicecare and communicate with our pharmacy provider
- the role, as part of a multi-disciplinary team, supports in the delivery of high-quality palliative and end of life care, through giving information and advice, training, and education
