care coordinator
Responsibility
- utilise population health intelligence and clinical systems to proactively identify and work with patients eligible for the pilot
- encourage patient participation in the pilot
- support and participate in shared learning within the PCN
- job responsibilities Working under the direction of the PCN Manager and the Clinical Lead for the Pilot and as part of the wider PCN and Pilot teams, the post holder will be responsible for administration of the Liver Surveillance Pilot, as requested by the management team, Primary key responsibilities
- raise awareness of cancer Symptoms and provide education to patients
- communication and Key Working Relationships Patients, relatives, carers and the public
- practice & PCN teams Liver Surveillance Team Health & Wellbeing Team
- work with member practices in health promotion Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care; Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level; Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; Support the coordination and delivery of meetings as required
