Responsibility
- we are seeking a compassionate and reliable individual to fill the position of Care Coordinator with a growing team
- assist care workers with their day to day rostering schedules and ensure they are matched based on clients preferences and skills
- we are currently seeking a part-time/full time Care Coordinator to join our expanding team
- this position will be responsible for assisting service users with their daily needs, ensuring they are comfortable and well cared for
- manage rosters on a weekly basis to ensure care staff capacity is made available and service delivery is not affected
- liaise and communicate with multi-disciplinary teams such as social services, GPs and Nurses in relation to each service users needs
- liaising with social services for new referrals and uploading new service users on a care management system
- communicate with service users/NOK's and care staff to ensure person-centered care is provided and maintained while promoting service users independence
Requirements
- must have a current CRB/DBS that is up to date and relevant training
- must be able to communicated professionally and effectively both written and verbal
- must be able to have a full driver's license and have own transport
- be able to work in multi-task environment, be structured and organised
- must be competent in IT, adapt and learn new software systems especially care management
- must be able to work as a team, be caring by nature
- must have good attention to detail, be able to solve problems when they occur
- must have at least two years experience working as care coordinator
