Responsibility

  • rostering staff to provide a personalised, safe and caring service to clients in the community
  • A coordinator is responsible for ensuring that the carers are efficiently and effectively coordinated to the clients in the community
  • to take joint responsibility for the provision of care with the coordination team
  • liaise with other professionals involved in the service users care
  • to support the On-Call service during out of hours
  • sunny lives Support Health and Social Care are seeking an experienced Care Coordinator to oversee the coordination of care hours for a busy domiciliary service in Southampton
  • this will involve speaking to both clients and carers to ensure that the care being delivered by the domiciliary care assistants matches the expectations of the client

Requirements

  • experience within the care sector, preferably homecare / domiciliary care
  • experience working with carers to maximise outcomes for the clients
  • familiarity with CQC requirements
  • experience as a Care Coordinator
  • level 3 NVQ in Health and Social Care
  • providing care: 1 year
