Responsibility
- assessing the needs of the family carer and taking a solution-focused approach
- liaising with other professionals to ensure any identified needs are met
- to provide in-depth information, advice, guidance and emotional support to family carers in all communication formats
- to actively listen to family carers, gaining an understanding of their caring role and the impact it has on them
- to flexibly work across the services, within reasonable expectations, as and when required, to support business need
- to work closely with, and refer to, other Suffolk Family Carers services, as required
- providing relevant and up-to-date information, advice, guidance and support to family carers on a one to one basis in their own homes, via the telephone or suitable environment
- to signpost or refer family carers to organisations for further support and information
