Responsibility
- the successful candidate will need to have strong people skills that equip them to deal with difficult situations
- work with the GPs, Long Term Condition Nurses and other primary care professionals within the Practice to identify and manage patients
- the following are the core responsibilities of the role holder
- this will involve the use of the Quality and Outcome Framework reporting tool and Enhanced Services
- full training will be given in all aspects
- you will be required to work some Saturdays to provide the service as required for certain projects
- support GPs in the delivery of patient care and follow up
- flexible hours on part time or full time basis and remuneration to be discussed at interview
