- to contribute to the strategic, clinical and operational business planning for the organisation
- PLEASE SEE FULL JOB DESCRIPTION ATTACHED To operationally manage the St Lukes Hospice Patient and Family Support Service; this delivers social work, social care, spiritual and bereavement care, befriending services via the inpatient unit and in peoples homes
- to line manage a team of social workers, a social and bereavement support worker, children and families support worker and a team of befriending and bereavement volunteers To manage a referral system for triaging, risk assessing and allocations process
- to align referrals and caseloads in line with the 4 tier model of social care To support the Deputy Clinical Director and senior management team in the development of strategies related to social care
- to act as an expert social worker and social care resource for palliative care, providing support and advice to patients with progressive life-limiting disease, their carers and families
- advising multidisciplinary colleagues and professionals from other agencies to ensure consistent access of patient and families to all levels of social care support
- to hold a small caseload of complex cases as well as providing cover for colleagues when on planned or unplanned short-term leave
