  • carrying out client reviews and risk assessments
  • follow up on all concerns, referrals and liaise with other professional in a Multidisciplinary integrated care setting

  • supervisions and monitors Caregiver's progress i.e. observation/ spot checks
  • we are looking for an individual with the strength and determination to support us in taking our business to the next level for a handsome reward package
  • participate in pre-on boarding client assessments and complete care plans
  • the applicant must drive or have driver's licence with good communication skills and at least 2years experience in care planning and monitoring in the UK
