develop relationships across the PCNs, ICB/ICS and other service delivery organisations, patients and communities to plan and deliver changes that will improve working between general practice and other primary, community and wider care services
lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN, ICB/ICS to agree changes, share learning, discuss challenges and resolve issues

Responsibility
copeland Primary Care Network are introducing this role into their current leadership team to enhance the strategic operational management alongside recognising the requirement to recruit a person who has the required digital understanding and vision with which to lead the PCN digital strategy
the post holder would be expected to work collaboratively with our Clinical Director and Transformational Clinical lead to support delivery of high quality healthcare across Copeland Primary Care Network covering 6 GP practices and a population of 62,500 residents
the post holder will lead the development of a strategic plan that includes a robust digital infrastructure taking into consideration the populations health needs and health inequalities
the post holder will be expected to clarify the strategic objectives and priorities for the Primary Care Network delivery in line with National and Local requirements maintaining oversight and accountability for the delivery of local incentive programmes, PCN contracts and services
maintaining overall responsibility for the management of the network working within operational capacity, financial parameters and contractual requirements working closely with the PCN Clinical Director and the PCN leadership team to achieve success
to have accountability for the smooth and safe running of the PCN and be the first point of contact for the Practice Managers within the PCN with regards to projects and schemes
to provide oversight and accountability to the delivery of PCN contractual requirements and specifications
develop and implement mechanisms for assessing the impact of agreed changes
