- the duties of this post will be subject to regular review, at least on an annual basis
- be a prescriber, or commit to completing training to become a prescriber
- support colleagues and patients with medication queries and monitoring
- take responsibility for the medicines management of some patients with complex chronic diseases
- undertake Structured Medication Reviews to ensure patients take their medications correctly and get the best from them, reducing waste and promoting self-care
- undertake clinical medication reviews to proactively manage people with complex polypharmacy
- to work with patients who need support reducing long term prescribed opiates
- contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, recommendation of medication changes and communication accordingly
