Responsibility

  • position: Clinical Pharmacist
  • hours : Negotiable
  • reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review
  • conduct structured medication reviews to proactively manage patients with complex polypharmacy
  • manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates
  • manage patients and health care professionals medicine queries
  • provide clinical medicines expertise to both patients and practice teams
  • improve prescribing practices across the Practice and PCN

Requirements

  • pharmacist: 1 year
  • working within a GP Practice: 1 year
