Requirements
- work shifts on a 24/7 basis as required by the home
- ability to understand, analyse and develop positive behaviour support strategies for young people
- ability to confidently communicate with parents/carers of young people using the service
- knowledge and experience of supporting children with a disability would also be an advantage
- ability to work effectively within a large team and enjoy the changing pace of a Short Breaks and outreach service
- ideally, an NVQ level 3 in health and social care for children and young people or be working towards achieving NVQ Level 3 in Health and Social Care and Children and Young People’s Services., or a qualification in social care, nursing, education, which would meet the registration requirements
- previous experience of being part of a key worker/care team
- experience of helping disabled young people overcome social inclusion barriers
Responsibility
- by taking a lead with case/care planning management for individual children or young people, undertaking assessment of need, developing a care plan and identifying agreed outcomes and carrying out regular review in conjunction with children, young people and their families and other professionals
