Responsibility
- support people in accordance with their needs and wishes as detailed in their person-centred support plans
- lead and act as the first point of contact in the absence of the Resource Manager
- ensure that support plans reflect the changing needs and wishes of the clients by contributing to regular monitoring and review
- take responsibility for full and accurate communication at handover
- accurately complete documentation, including clients’ records
- encourage and enable the clients to become active members of both their local and wider communities
- support people to develop and maintain good family relationships and a range of friendships
- compile team rotas and activity plans as required by the Resource Manager
Requirements
- passionate about making a difference and the work of Integrity Social Care Solutions Ltd
- experience of supporting people or of working in a social care setting
- SVQ Level 3 in health & social care or equivalent
- to achieve SSSC registration and undertake the required qualification within the given timescales
- this post is subject to a PVG Disclosure check
- clear written and verbal communication style
- basic IT skills and numerical ability
- willing to undertake required training, sometimes off site and outside normal working hours
