HORIZON TRANSITIONAL CARE is looking for employees for positions:

residential support worker

Working hours

Responsibility

  • to support the young people by acting as an internal advocate, involving them in every aspect of their care
  • support in the arrangements for contact and ensure that the young person’s wishes and feeling are taken into account
  • completing weekly key work sessions with allocated key child or other young people within the home. Key work sessions to be recorded and shared with the young people
  • organising and participating in appropriate activities
  • fulfil sleep-in duties in accordance with the agreed rota by the home manager
  • administer medication in accordance with Company and Regulatory Body policies and procedures. Ensure that medication is stored in safe manner in accordance with policy
  • to carry out any other reasonable and relevant duties as required by the management of the home and wider management of the organisation
  • to work to and exhibit the POSITIVE values of the Company and maintain standards of behaviour in accordance with Company policies, procedures and practices

  • SEN experience and able to use effective and creative resources to plan and support identified progress of children and young people with SEN needs
