Responsibility

  • PRIMARY DUTIES AND RESPONSIBILITIES Provide weekly clinical supervision to the junior pharmacists
  • support junior pharmacists with more complex clinical queries that arise from their individual practices
  • lead in the delivery of the PQS audit activity
  • operationalise and regularly review the work streams for the pharmacists team
  • manage day-to-day medication related queries coming through the Practice via telephone and online, from patients, community pharmacies and hospital pharmacies
  • reconcile medications in patients discharged from hospital who have had multiple medication changes, and organise appropriate follow up if needed
  • regularly review CQC medications and prescribing related searches
  • triage medication review forms completed by patients via the practices online consultation platform
