- provide guidance and support to clients and their families regarding healthcare options, resources, and financial considerations
- assist clients with medication administration and management
- monitor and evaluate the effectiveness of care plans, making adjustments as necessary
Requirements
- bachelor's degree in social work or a related field
- previous experience in home care or a similar role is preferred
- strong knowledge of budgeting and financial acumen
- excellent communication and interpersonal skills
- ability to work independently and as part of a team
- compassionate and empathetic approach to client care
- proficient in using computer systems for documentation purposes
