Requirements

  • understanding of NHS long term plan and priorities relevant to Primary Care
  • capacity to be innovative and develop the role of the Care Co-ordinator
  • ability to work with a range of clinical and non-clinical personnel as part of a team
  • ability to work independently and effectively with a high degree of motivation
  • able to prioritise and work to deadlines
  • ability to communicate information to patients and carers in an appropriate manner, using well developed empathy skills
  • understand the current issues facing primary care team

Responsibility

  • escalate any issues where appropriate to the Strategic Manager
  • co-ordinate the work of the clinicians and any non-clinical staff involved with the care of any patients registered with the practices within the PCN

Responsibility

  • this is not an exhaustive list of duties and responsibilities but indicates the key responsibilities of the post
  • the post holder may be required to undertake other duties as may reasonably be required of you commensurate with your grade and/or hours of work at your place of work
  • carry out any other duties as may reasonable be required by the Clinical Director PCN Board and its members
  • to take reasonable care for the Health and Safety of themselves and of other persons who may be affected by their acts or omissions at work
  • manage financial requirements on behalf of the PCN
  • collate feedback / analysis data on behalf of the PCN to report to the CCG and other organisations as required
  • the post holder will be responsible for providing operational management support to the practices within Acclaim PCN

Requirements

  • minimum of 12 months experience in a senior role working with/in Primary Care or similar with transferable skills
  • clear evidence of managing financial accounts procedures and budget control
  • experience of working on GP clinical systems
  • demonstrate a wide range of leadership qualities, skills and behaviours
  • experience of developing and implementing common systems and processes across teams and across service boundaries
  • ability to plan, organise and co-ordinate work effectively under their own initiative
  • experience of facilitating and managing change
  • experience in managing and leading staff
