- BASE: Hybrid working - Post holder will be expected to work in different locations across the borough and work from home
- MANAGED BY : City and Hackney Office of Primary Care Networks - Operations and Programme Director
- JOB SUMMARY: To support the design, planning and implementation of the City and Hackney Personalised Care Strategy
- maintain oversight of relevant budgets, monitoring expenditure against targets, ordering equipment, supplies or training, complying with Standing Financial Instructions within designated remits
- using quality improvement methodologies and drawing from the latest Personalised Care guidance and good practice to support the implementation of projects across the Neighbourhoods
