JOB TITLE: Personalised Care Implementation Manager
GRADE: Band 7
LENGTH OF CONTRACT: 12 months
BASE: Hybrid working - Post holder will be expected to work in different locations across the borough and work from home
MANAGED BY : City and Hackney Office of Primary Care Networks - Operations and Programme Director
JOB SUMMARY: To support the design, planning and implementation of the City and Hackney Personalised Care Strategy
maintain oversight of relevant budgets, monitoring expenditure against targets, ordering equipment, supplies or training, complying with Standing Financial Instructions within designated remits
using quality improvement methodologies and drawing from the latest Personalised Care guidance and good practice to support the implementation of projects across the Neighbourhoods
