Responsibility

  • coordinating and scheduling care services for clients in their own homes
  • supervising a team of care assistants and ensuring that they are providing the highest quality of care to clients
  • communicating with clients' families and healthcare professionals to ensure continuity of care
  • assessing clients' needs and developing care plans to meet those needs
  • we are seeking a highly organized and compassionate Care Coordinator to manage the care and support of individuals in their own homes
  • monitoring clients' progress and making adjustments to care plans as necessary
  • the successful candidate will be responsible for coordinating and supervising the delivery of care services to clients, ensuring that they receive the highest quality of care
  • ensuring that care assistants are following care plans and reporting any changes in clients' conditions
