  • ● Experience of delivering home care services to people living in their own homes
  • ● Experience of supervising a dispersed staff team
  • this includes Care notes, additional documentation, quality monitoring forms, spot checks, referrals, assessments and care plans
  • ● Take details of new client referrals and arrange for worker allocation, entry of details into IT system and prepare care notes for the care package in preparation for the package commencement
  • ● Carry-out on call shifts as required in our out of hours’ service. Coordinators will be required to carry out one on-call shift per week and if required to assist to cover the weekend on-call on an ad-hoc basis in situations such as sickness
  • ● Prepare all documents for service user files and work with the assessment team and designated assessor to ensure service user files are complete
  • carry out initial investigations for any complaints raised in specific areas and provide written documentation to the Complaints and Safeguarding Manager as required

Responsibility

  • ● Carry out the on-site observation and evaluation of care worker practice as part of routine supervision and spot checks, in response to performance concerns, or as otherwise directed
  • ● Carrying out regular and unannounced spot checks on Care workers and service users
  • ● Ensure all documents and files are completed and CQC compliant
  • ● Act as a point of call if the service user has any complaints or issues and ensure these are escalated and reported to the Office
  • ● Where necessary, introduce new care workers to service users

Responsibility

  • to provide a personal care service to service users to help to enable the individual to lead as much of an independent lifestyle as possible
  • the individual will be required to follow and work with the provided care plan to ensure that the needs of the service user are met
  • healthcare: Prompting and assisting with prescribed medication as required. This does not involve administering medication

  • manage all elements of the care package ensuring any changes or issues are communicated with the relevant parties
  • amending the package as and when deemed necessary
  • weekly management of designated rota for both Care workers and Service users and ensuring Service users or next of kin are updated of any changes to the rota
  • taking on new referrals and ensuring they are added to the relevant system ; it is also the responsibility of the co-ordinator to communicate with the Finance Department the details of the package and advise should there be a need to credit or not
  • ● Carry out care worker supervisions and service user quality monitoring and provide written documentation of this and ensure these are added to care worker files
  • ● Ensure that any changes are immediately communicated to the care coordinator and care workers and if applicable Service users
  • ● Record emergency calls and pass the information to appropriate parties
  • ● Promote good relationships with Care workers and Service users

Requirements

  • ● Experience of delivering home care services to people living in their own homes
  • ● Experience of supervising a dispersed staff team
  • ● Experience in a similar role with a minimum of 2 years’ experience of managing 1,000 hours per week
  • ● Be a true leader who can lead by example and motivate a team
  • ● Be well presented
  • ● Hold a minimum Qualification of QCF Level 2 or 3 in Health & Social Care
  • ● Excellent telephone manner
  • ● Good attention to detail
