Responsibility

  • senior Healthcare Team Leader
  • you will be a key part of the transition to a new home for the person in the local area
  • we are in the process of building up this young person’s team and need a Senior Health Care Team Leader to understand the values of person centred care and support towards the person
  • you will be leading the Team in support of the Manager and Case Manager and you will be trained to work in a consistent way and be expected to change your behaviours to meet the person’s needs
  • identifying those triggers through reflective practice significantly reduces the huge amount of unnecessary stress for the person
  • the young person is Autistic and, through no fault of his own, is now a permanent wheelchair user needing personal care
  • we need an enthusiastic, skilled and well trained Senior Health Care worker to join a team who will use person centred guidance in order to make it a reality in any situation. You will need to be computer literate
  • you will be a Role Model for what needs to be done, both in the home in Stanford le Hope and when he is out in the local community, based on the guidance provided by people who know him best including other Professionals who have worked with the family
