Responsibility
- conduct needs assessments with clients and their families to understand their health care requirements
- develop and implement care plans in collaboration with clients, their families, and medical professionals
- coordinate and monitor services provided by health care professionals and home health care agencies
- provide education on disease management, nutrition, and medication management
- support clients through emotional or behavioral changes and provide appropriate resources and referrals
- maintain accurate client case files and progress notes
- monitor the progression of client care plans and make necessary adjustments based on the client’s needs
- connect clients with community resources and support groups
Requirements
- bachelor's degree in health care, social work, nursing, or a related field preferred
- minimum of 2 years of experience in care coordination or home health care required
- understanding of clinical processes and standards of care
- demonstrated ability to coordinate medical services and manage multiple care plans
- ability to work in a fast-paced environment and manage multiple priorities
- excellent communication, interpersonal, and customer service skills
- strong organizational and problem-solving skills
- ability to work independently and as part of a team
