ARMSTRONG AFTER CARE SERVICES is looking for employees for positions:

care coordinator

Working hours

  • full-time | Contract | Permanent

Responsibility

  • conduct needs assessments with clients and their families to understand their health care requirements
  • develop and implement care plans in collaboration with clients, their families, and medical professionals
  • coordinate and monitor services provided by health care professionals and home health care agencies
  • provide education on disease management, nutrition, and medication management
  • support clients through emotional or behavioral changes and provide appropriate resources and referrals
  • maintain accurate client case files and progress notes
  • monitor the progression of client care plans and make necessary adjustments based on the client’s needs
  • connect clients with community resources and support groups

Requirements

  • bachelor's degree in health care, social work, nursing, or a related field preferred
  • minimum of 2 years of experience in care coordination or home health care required
  • understanding of clinical processes and standards of care
  • demonstrated ability to coordinate medical services and manage multiple care plans
  • ability to work in a fast-paced environment and manage multiple priorities
  • excellent communication, interpersonal, and customer service skills
  • strong organizational and problem-solving skills
  • ability to work independently and as part of a team
