Responsibility

  • for these roles you must have prior experience of Community Care, Care Co-ordination and a working knowledge of the care industry
  • co-ordinators will be allocating calls, processing new referrals and changes to clients care and support plans
  • work with the office team to maintain up to date electronic/hand written records
  • carry out client reviews on site and by phone
  • to carry out spot checks and supervisions, and assist the other co-ordinators with aspects of their role
