- RESPONSIBLE TO: MDT Team Leader
- to help strengthen community and personal resilience and reduce health inequalities, particularly for people with long term conditions, are lonely or isolated, or have complex social needs which impact on wellbeing
- 17. Attend any training courses and supervision sessions as required
- 16. Work in accordance with practice policies, ensuring that all work and procedures are undertaken in accordance with issued guidelines
- 19. Any other duties that may be required and agreed by the Doctors and Management Team
- RESPONSIBLE FOR: Empowering patients to take control of their health and wellbeing taking a holistic approach, connecting people to community groups and other services for practical and emotional support
- where required and as appropriate, refer people back to other health professionals within the network
- 15. Attend all meetings as required
