Responsibility
- team Leader for days required for well established and respected step down facility
- the Grange is a 17 bed step down and reablement facility, supporting discharges from community and general hospitals
- \* To support patients upon discharge from hospital within the step down facility
- we offer short term reablement to assist patients back onto their feet and hopefully return home
- \* Plan and introduce an effective and robust rehab plan for patients living with prolonged health concerns
- shift pattern comprising of 12 hour shifts on a 2 week rolling rota, alternative weekends
- \* Partake in the admission and assessment of patients from community and general hospitals within the region
- \* Partake in the safe discharge of patients home or to other support facilities
