- job Summary The Young Peoples Social Prescriber Apprentice will work closely with other Social Prescribers across the East Riding, working with general practices and some schools to support young people aged 11-17 to help tackle loneliness, isolation, anxiety and depression by connecting them into a broad range of services & interventions to improve their health and well-being
- together produce a simple personalised care and support plan to address their health and wellbeing needs
