  • this role sits as part of PCNs team of health professionals that take referrals from the PCN and a wide range of agencies delivering the social prescribing plan as determined by the PCN, Commissioners, social prescribing schemes, Local Authorities, and voluntary sector leaders

  • job Summary The Young Peoples Social Prescriber Apprentice will work closely with other Social Prescribers across the East Riding, working with general practices and some schools to support young people aged 11-17 to help tackle loneliness, isolation, anxiety and depression by connecting them into a broad range of services & interventions to improve their health and well-being
  • together produce a simple personalised care and support plan to address their health and wellbeing needs
